The following documents are needed to complete registration: 
Your Child's Birth Certificate
Social Security Card
Immunization Record 
One Proof of residence (utility bill w/guardian name/address, copy of lease agreement, property deed)
Registration Form (please print, complete & scan to email)

May 25th & 26th @ 8am-2pm
If you registered online at home…please bring your child to
HPS one of these dates for a quick Kindergarten skills screener.
You will need to speak to someone in the office if any of the following scenarios apply to you:
 There are custody papers or a no contact order. (Please have copies on-hand of orders)
 The Utility Bill is not in one of the Guardian’s name.
 Your child has an epi-pen, a major medical condition, or will take medicine at school.
